NEW CLIENT

Required before Body Torch moves forward with your Activation process.
1. Client Self-Service Agreement
SIGN SELF-SERVICE AGREEMENT2. Client Waiver & Release
SIGN WAIVER & RELEASEIf either form is missing: no order release, no individualized protocol release, and new appointments may be cancelled. Activation Session deposits remain non-refundable.
CURRENT CLIENT
Complete 1 Form
If you are already an established Body Torch client, the Client Self-Service Agreement must be on file before any new order is released.
SIGN SELF-SERVICE AGREEMENTIf the agreement is not on file: your order remains on hold until the signed agreement is verified.
You do not need to complete the general new-client Waiver again solely because you placed another order.
Use the same email address you use with Body Torch so your signed forms can be matched to the correct client record. Body Torch verifies your status and required documents before releasing the next step.
